DHEC Permit Number & most recent DHEC inspection report
Certificate of Insurance (COI)
ServSafe Manager’s Certificate
ServSafe-certified manager onsite during production
Completed SC DHEC 1769 form (specific to the Feed & Seed Community Kitchen) with payment receipt
Inspection performed at the Feed & Seed facility
